Sending Tier II Reports to the LEPC and Omaha Fire Department Special Operations Division


Facilities in Douglas County subject to the EPCRA must submit Tier Two Emergency and Hazardous Chemical Inventory forms on an annual basis to the Douglas County LEPC and to the Omaha Fire Department Special Operations Division. This can be accomplished by using the following methods. 

  1. Scanning and emailing the forms to: This email address is being protected from spambots. You need JavaScript enabled to view it. , or by
  2. Sending a hard copy to: Douglas County LEPC c/o DCEMA, Civic Center EOC, 1819 Farnam St., Omaha, NE 68183, and to the Omaha Fire Department Special Operations, Omaha Public Safety Training Center, 11616 Rainwood Road, Omaha, NE 68140.

In addition to the guidance stated in paragraphs “a” or “b” above, facilities should also ensure that Tier II forms are sent to the Nebraska Department of Environmental Quality, Lincoln, Nebraska.
